To begin the process, simply download and complete both the survey and the application. Once completed you can either:

1. Email the forms to Pupswithpurpose@forsythco.com 

2. Deliver them in person to the Forsyth County Jail at 202 Veteran's Memorial Blvd., Cumming, GA 30040

• Please note that submitting an application is only the first step in our process and does not guarantee the adoption of one of our Pups with Purpose.

• A mandatory Meet and Greet is required before the adopter can move on through the adoption process.

• If the potential adopter has other dogs, they will be required to bring them to the meet and greet to ensure this will be a good fit.

• The $85 fee to finalize the adoption is paid at the FCSO Pet Adoption and Resource Center located at 4065 County Way, Cumming, GA 30028.  Additional donations can be made directly to the Pups with Purpose program.

• As part of the adoption, you receive 5 weeks of beginner training with Rucker Dog Training.  These session will show the future adopter how to train the dog.  They will learn all the commands the dogs currently know.  They will also be given a pamphlet that shows the adopter what food the dog eats, toys they enjoy playing with, treats they prefer, and what products we use on the dogs.

• The RSAT/Pups with Purpose Deputy will call the potential adopter's vet to make sure all State required vaccines are being maintained for their current pets as well as making sure they are on prevention medications. 

• Selected applicants are subject to a background check. Every adult in the selected home will need to submit the background check paperwork and a send a copy of their driver's license to complete the background.
